The Fire Works are found at the Body Shop.
The Green Hat is found at the Town Square.
The Party Hat is found in the Garage.
The Flowers are found at the Park Room 2.
The Present is found at the Space Port.
Once you've collected all the items go back to Dr. Frakenbody and talk to him you should get a reward!
